We accept patients from age infant to >100 years old. We offer management of acute as well as chronic issues ( COPD, ASTHMA, HYPERTENSION, DIABETES, , CONGESTIVE HEART FAILURE etc.). Services Included but not limited to WELL CHILD CHECKS, SPORTS PHYSICALS, IMMUNIZATIONS, WELLNESS PHYSICALS , PAP SMEARS, STD SCREEN, PLANNING FOR PREGNANCY COUNSELING AND EDUCATION, etc.).

Primary care services offered in the homes for those who are home bound as well as patients residing in long term care facilities.
